One thing I have found useful in following up with the families is phoning them two weeks after their last appointment and seeing how they are doing. Sometimes it just seems that a month is a long time to go by between visits, and it gives me the chance to update myself on recent happenings in the family. Having a notebook handy to record these things is also helpful.
